We need you to upload some documents and provide us with some information. The documents you'll be needing are:

Professional indemnity insurance certificate of currency
Letter from Medicare with your provider number for ACT of Living (if you don't have this now, you can still complete the other requirements now)
Bank statement
Recent curriculum vitae
Head and shoulders photo of you

This should cover everything we need you to provide for us so we can set up your webpage, billing and clinical tools at The ACT of Living.

If you have all the above documents ready, this should take you 10-15 minutes. But don't worry if you don't have a document now - you can save your progress and move on to the next step, then return to this portal when you have the missing documents.
